Form can be printed and completed but must be attached to a physical green form with "See attached completed form" written in the explanation section of the green form.

Form can be printed and completed but must be attached to a physical gold form with "See attached completed form" written in the explanation section of the gold form.

Bible Light Facility Usage

Member(s) wishing to use church facilities, and/or equipment, must complete a Facility Usage Form and return it to the designated person at least three (3) weeks prior the date of usage.
